(C) illiteracy costs the United States at least $20 billion a year in lost industrial output and tax revenues
(D) $20 billion a year in lost industrial output and tax revenues is the annual cost to the United States of illiteracy

The expression is "the cost IN ____"

When you're talking about how something is affected other than in terms of cash.

Examples:

The cost of the new budget plan in lost wages is huge.

The declining economy has cost the country much in consumer confidence.

Consider, what is decreasing? "the cost in wages", "the cost in confidence". It's describing the currency of the cost.
