What are recommenders asked?

I think this is somewhat of a black box to a lot of people. I know for myself I've been wondering lately, what recommenders are asked. I know the types of questions they are asked, but can someone shed some light on:

1. School
2. Exact questions recommenders were asked?
3. Word limits

For most of the schools you can download a pdf of the recommendation form (from the school website) in case your recommenders can't submit electronically and have to mail it in. I wonder how much longer that will continue given everything else is electronic though.

From what I can tell, they ask a range of short answer questions and then also have the recommender evaluate the applicant on a range of criteria. I didn't apply to Harvard/Stanford/Wharton so I don't know what those are like.

If you do a google search and poke around online you should be able to find the documents.
