I just downloaded the error tracking tool for Official Guide, edition 12 and solved a few problems and all the answers were displayed as wrong in the excel, but when I checked the answer key in OG, my answers were right.
to quote an example, the review excel says the answer for question 2 of problem solving in the diagnostic test as B, but the book says D and I too find D as the correct answer.
