The dog has bitten my younger son twice, and each time, he has had to
be sent to his kennel.

A and B are wrong because HIS kennel
E is wrong because HE has had to be sent

C and D we shoud choose between Each and Every.
Since in initial sentence we have each time I prefer to remain this constraction and my answer is C

IMO C.

'each' is used in case of two things and 'every' in case of more than two. So B and D ruled out. E is wrong because it uses two different pronouns 'he' and 'it'. And since we are talking about the dog, I think the required pronoun is 'it' and not 'he', as given in A. So the answer should be C.
