This is quite an intriguing question. I picked [spoiler][D][/spoiler], because it correctly modifies "the ailing mobster" and "the town"

My question to those who are convinced that OA is [spoiler][A][/spoiler] is don't you think that "near to those he most trusted" incorrectly modifies town; instead the correct sentence- "near to those" must modify town, but "he most trusted" must modify the gangster.
