IMO A

A correctly captures the past perfect continous tense "had been" for past actions continuing. E has "their" incorrectly to refer to the company.

IMO B,
mismanaging--indicates continuing act while for past i think it should be..

its many irregularities had been the long-term [mismanagement of funds..]
Wounded by GMAT but not dead
