Between X to Y is incorrect IDIOM. Therefore, (A) is incorrect.

(C) Tracy Austin, the tennis celebrity who, plagued by injuries, won the Italian and U.S. Opens at age 16 in 1979, sat out from
1. (C) is wrong because the comma after WHO and comma after INJURIES are unnecessary.
2. If we remove the comma then the meaning of the sentence is distorted. So, to retain the meaning we need comma over here. But commas instead of making the sentence simple, introduces further ambiguity to the sentence.
3. In (C), it is unclear that whether she won the title during injury or after injury.

Check the both option below. (D) has very clear meaning over the (C). It simply states that injury occurred after winning the title that force her to do commentary instead of playing further tennis.
Incorrect Option wrote: Tracy Austin, the tennis celebrity who, plagued by injuries, won the Italian and U.S. Opens at age 16 in 1979, sat out from 1984 to 1988, occasionally working as a television commentator.
Correct Option wrote: Plagued by injuries, tennis celebrity Tracy Austin, who won the Italian and U.S. Opens at age 16 in 1979, sat out the years from 1984 to 1988, occasionally working as a television commentator.

but shouldn't modifier be placed like this...
i Think the usage of modifier in D is incorrect...
