(A) and their families have a history of high blood pressure...breaks the flow of the sentence.
(B) whose families have a history of high blood pressure
(C) and a history of high blood pressure runs in the family...awkward
(D) whose families have a history of high blood pressure running in them...
(E) with a history of high blood pressure running in their family...'and' missing

I will go with E, but if and was not included in the underlined portion, and whose families would have sounded more correct.

Ans - E

Reason - " and with" clearly describes the family of "people"

IMO B, whose is aptly modifying young people (whose can modify only people or places)
E -> with a history of high blood pressure running in their family???
blood pressure can't run in a family-awkward
