1) I have had success working as a nutritionist. - Correct
2) I had had success working as a nutritionist before I joined the hospital. - Correct

3) I have success working as a nutritionist. - Incorrect

This is a tense issue:

Similar sentences:

Though I had written a book on finance, I hoped to write on accounting.

---> Here, I have already written the book, and I am hoping for another.

There are two past actions. One of them had occurred before the other. So, the use of past perfect is perfect in this case.

What about C?

He had success---it is past simple, but we need past perfect. So, c is out.

Answer is A.

#3 isn't incorrect, it's a general statement. Compare:
I have had a good time going to birthday parties. -- Statement summarizing multiple past events
I have a good time going to birthday parties. -- Statement of general truth

I'd have chosen A as well because of tense as well,

Yeah, I agree about the general truth statement. It's just that I have success... as a general truth sounds really weird to me.

time frame 1: till the success, before opting out.
time frame 2: the opting out and after.
