Understand the difference
One of the factorS THAT ARE responsible...
Here the use of that is done to refer to factorS and thus we use ARE. because we are talking something that is applicable to all the factorS not any particular factor.

One of the factors responsible for destruction IS xxxx
Here we don't have THAT and we are talking about that one factors out of various factors.
