It was decided by us that the emphasis would be placed on the results that might be attained.
A. It was decided by us that the emphasis would be placed on the results that might be attained
B. We decided that the emphasis would be placed on the results that might be attained
C. We decided to emphasize the results that might be attained
D. We decided to emphasize the results we might attain
E. It was decided that we would place emphasis on the results that might be attained

I thought it's D, but the right one is C
What's wrong with D?

Please check the original sentence.
You must have reasons to dismiss this answer
It was decided by us that the emphasis would be placed on the results that might be attained.

we require only one that, and so the presence of another that makes the sentence redundant.
B. We decided that the emphasis would be placed on the results that might be attained
Similarly B and E is also redundant

Now left with C and D
D. We decided to emphasize the results we might attain
we need THAT to introduce an essential clause9but only 1 that)

So answer is C

Hope this helps

Mayur, D also alters a meaning by assuming that 'we' are the ones you will be attaining the results. C doesn't.
