1) What score did you get?
2) What books did you use?
3) What prep courses did you take (if any)? Full length, weekend?
4) How long did you study for, and under what conditions? (during school, during the summer, full time work, etc)
5) How many preptests did you do - feel free to comment on score progressions, digresssions or plateaus?
6) What would you change if you were to do it again?
7) How many times did you take the GMAT?
8 ) Is you background (work/undergrad ed) quant or verbal focused?
9) Any other misc comments/suggestions

Saw a similar survey on another forum (but for the LSAT) and found it very informative and interesting to compare and contrast how ppls responses differ when asked the same questions...hopefully ppl will fill out the survey and this can be added as an additional resource for those just starting out...

I am not sure whether you will get a good response in this section. People generally post their story in I just beat the GMAT section. You may look there for individual success stories.
Wish you good luck
