Hello,
Overall I consider that your strategy should be to apply to programs that (1) are in the mid-range, where your stellar GMAT will stand out as a deciding factor AND that (2) will meet your interests and needs. It looks like you’re already applying this strategy basically. To answer your specific questions:
1. You probably will have to address it, as many applications ask why you left previous jobs. You can address it succinctly and straightforwardly in an optional essay. Also make sure that your recommenders absolutely glow about you, to counteract the effect it might have.
2. Georgia and Kansas are good targets. The other schools I think are probably out of reach, as you will need some track record of success and a coherent work story for all of them.
3. There are many excellent schools in your range – look at Univ of Minnesota, Baruch (CUNY), etc. I can’t recommend specific schools without knowing more about your goals.
4. Good and bad. It clearly indicates an ability to handle rigorous coursework, that’s the good part. The bad part is that in the context of the application overall it could seem unfocused. You can use your essays to weave the threads together as coherently as possible.
