Is it E.

Possible and May are redundant in the sentence. A,B and C can be eliminated.

B/w D and E. "melting of the polar ice" is more appropriate than "melting the polar ice". Choice E.

The word possible makes the usage of may appropriate in this context. Article "the" is always use with geographical features and natural occurrence. examples are 'the Atlantic Ocean', the depletion of Ozone layer', the Amazon.
( melting the polar ice caps and a rising sea level in this question is a natural occurrence that is applicable to the 'greenhouse effect'). Thus, article "the" will fit in perfectly to the usage of these effects.

Option B is correct.
"may include the melting of polar ice caps and the rising sea level"
