Doubt ... isn't OA changing the meaning that ...the ancient civilization flourished as "the civilizations in the Nile Delta and the river valleys of the Tigris and Euphrates."

Hey no, it does not alter the meaning.
Here ancient civilization is compared to the civilizations in Nile and Tigris. The parallelism is maintained in A.

B) unnecessary use of past perfect. past perfect- had is not necessary.

C)that flourished at the same time those had ('Those'- pronoun reference error. 'Those' cannot refer to a civilization)

D)flourishing at the same time as those did (same error as C- pronoun referenc error)

(E) flourishing at the same time as those were (same as C- pronoun reference error)
