Some patients who do not respond therapies of depression may simply have received inadequate treatment, having, for example been prescribed a drug as a dosage too low to be effective or having been taken off a drug too soon.

Well b, c has the usage of being and is wordy,so eliminate.E says drug prescription with a dosage which changes the
meaning of the sentence completely.You are left with a and d.D again is wordy,when they have?or were...so would go
with A,the original,better among all.Really brutal indeed!
