In general:

1. August/September (ish)
2. September/October/November (ish)
3. R2 , R3 are so that people can apply at different times in the year for different reasons... ie. waiting for a promotion... taking or retaking the GMAT... just got started late in the process...

I think Money9111 answered most of your questions, I would just add that you are able to submit your applications as soon as the essay questions/application is released (summer). And, you should submit as early as possible in each round (if you miss the deadline for R1, you should still submit early in R2 rather than waiting for the deadline). Keep in mind that your submissions are not considered complete until the GMAT score and recommendations have been received! And, as you may hear on this board and elsewhere, the conventional wisdom is that it is better to apply in an earlier round!

There are a few schools (notably Columbia) who also offer January start dates (in addition to the traditional fall start date). January start dates usually mean school in the summer which can be difficult if you are hoping to have an internship.

Thanks for your answers. So the initial call for applications itself is R1 (basically there is no R0 etc) and applications are solicited for admissions ONLY into the following year not for year(s) folloiwng the next year. Thanks again. Time to GMAT now!
