(D) which, without permission, neither commoner or foreigner could only enter,
(E) which, to enter without permission, neither commoner or foreigner could do,
please explain what does B conveys and why it is wrong?

Besides being grammatically awkward, the word "only" is a word not at all present in the original wording (usually a red flag). It is redundant and adds nothing to the meaning of the phrase.

As mentioned by Feed always avoide extrem words (only, any etc), unless it's mentioned in all choices.

X or Y (should be parallel)
Without any permission (Redundant)
