Kleine-Levin syndrome, a rare neurological disorder primarily affecting adolescents, is characterized by cyclical episodes of extreme lethargy, heightened irritability, and appearing confused.

case of typical llelism we need apparent confusion to llel with heightened irritability and other part of list

We are showing the characteristics of the disease-hence the last part will be a noun.Appearing would denote a verb.
We are talking about a characteristic,not a person-hence 1,2,3,5 will be eliminated.

Hence ,answer would be D.

will go with D . parallelism is mainted [Adj][Noun] structure , which is inline with D

extreme lethargy , heightened irritability and ____________ apparent confusion. the only one tht fits well .

The only thing tested here is parallelism.

Hence , i'd bet on D :D
