Manhattan is correct here.

The error in the PFD solution as you have written it is that in each of the 6 options they provide, there are 2 possible ways for A and B to be inserted.

For example, in CDXX
It could be CDAB
It could be CDBA

So, for each of the 6 ways there are two options, 6*2 = 12.
