I found one reason to eliminate "D".
We can't use "she" after "and"!
Tanuj ran to the police and filed the FIR.( correct)

You can use a comma with a conjunction to link two independent clauses. In your example, the first one might be better since the actions are directly connected. But something like:

My student studied for three months, and he improved his score by 200 points.

would be absolutely fine.
