In (A) (C) (D) and (E) has not and probably means she has not driven for whatsoever reason only (B) specifically mentioned she has not been allowed to drive after the accident.

(A) has not been and probably never would be allowed to drive - Incorrect - 'Would be' is a conditional, it is not correct here. we need will in the second part of the sentence.

(B) has not been allowed to drive and probably never will be allowed to drive - Correct - we need 'been' to show ongoing process of not allowing the driving. Therefore, IMO this is correct

(C) has not and probably never would be allowed to drive - Incorrect - we need 'been' to show she is not allowed to drive is an ongoing process. Also, would is not correct here.

(D) has not and probably will never be allowed to drive - Incorrect - Same as C. We need 'been' in the first part.

(E) has not and probably never will be allowed to drive - Incorrect - Only difference between E and B is been. Since, we need been in the sentence, therefore, I have eliminated this option. Experts please comment on this option.
