Ans is D.

"to receive" is idiomatically incorrect in this case.

a)he created an avenue for the Caribbean nations to receive worldwide recognition
("he created" is redundant here)

b)created an avenue for the Caribbean nations in receiving worldwide recognition
("in receiving" is idiomatically incorrect)

c)creating an avenue that the worldwide recognition be received by the Caribbean nations.
(creating is not parallel to unified)

d)created an avenue for the Caribbean nations to receive worldwide recognition
(correct answer)

e)creating an avenue that the Caribbean nations should receive worldwide recognition
(creating is not parallel to unified)

Check the capital words.
It's sure B.

I think the parallelism is "Sir Garfield Sobers....unified and [he] created"

This leaves us with A, B and D.

I find "in receiving" unidiomatic because that guy created something with the aim that those people would receive recognition. This is usually expressed with an infinitive.

So, A or D.
I think "he" is superfluous and I would therfore chose D.
