Hi Micheal,
I am not an expert to give you advice but thought an independent view might help. Just as a critique I would say that probably you are not investing enough time on review.
This article tells you how to conduct the first level of analysis on an test:

And this article tells you how to conduct analysis on an individual problem (you should do this analysis on EVERY problem, whether you got it right or wrong!):

I came to know about these lately and believe that it should help.

Personally I don't think OG guide is anywhere close to sufficient for quant, and after doing all the MGMAT CATs twice, neither is MGMAT. Perhaps you might want to consider hiring a tutor, someone that may be able to identify your weakness and provide additional help in those areas.
