GMAT -660 (Q42 V39)
*4th attempt

Undergrad GPA - 3.4 (Health Sciences). Fair grades in quant coursework
Grad GPA - 3.3 (Physical Therapy)

Work Experience- 3 years U.S. Dept. of Veterans Affairs (2 promotions)
Board certified geriatric physical therapy

Extracurricular - leadership roles in several community/professional organizations

Planning to take 2 UC Berkeley Extension courses (accounting, finance, or microeconomics) to build quantitative profile.

Why MBA
Short term - Transition into management role at integrated healthcare system (e.g. Keiser Permanente, Geisinger, etc)

Long term - Entrepreneurship (would like to develop chain of health clinics to provide prevention/wellness services for the elderly)

Questions:
1. How much will admissions officers value the alternative transcript when evaluating my quantitative aptitude/ability to handle academic rigor?
2. Should I retake the GMAT and aim for higher quant score
3. What are my round 2 chances at Duke, HBS, Vanderbilt, Kellogg, Wharton, Michigan?

Well i am sure you are not interested in retaking GMAT, and it will be 5 th time, so leave it.

You are taking courses to prove your rigour, so that is fine because anyways your long term plans- short term plans will make it subjectively acceptable. YourGMAT quant score is a point to be addressed.

Your score is good for the top (10-20) . To be in the list you mentioned, you will need to put a terrific application for kellogg,wharton and michigan. hbs is technically a bit difficult, but then who knows, essays with good themes have done miracle.

Coming from an underrepresented applicant group, your GMAT score is sufficient to apply to all of the programs you listed. Given your healthcare background you have an excellent chance at Duke, Kellogg, and Michigan (with Vanderbilt as a true safety). Wharton is good target school and HBS a reach school.
You might want to apply through the Consortium (https://www.cgsm.org/universities/index.asp) to Michigan along with some other schools like Yale.

Also, research the Diversity pages of each of the school to which you are applying.
