Great question. It sounds like you can count this person as a peer just be sure she positions her recommendation in a way that highlights how you two worked together on the project.

My peer recommender does not have anything to write on the constructive criticism question on the peer recommendation letter. While he has no problems in writing the rest of the questions, and he came and asked me if he could simply state that he never provided such feedback to me.

What would you recommend - honestly state the fact, or provide a weak example that comes close to a constructive criticism?

Thanks for your post. In this situation it is probably better for your peer recommender to simply state that he has not encountered this situation with you rather than use a weak example just to feel like he's providing content.
