In geometry, a plane is defined as a flat surface with no thickness. Imagine the top of a table. The top surface of the table is a plane. Any objects lying on the table would be said to be "in the plane" of the tabletop.

Coordinate Plane
The plane formed by a horizontal axis and a vertical axis, often labeled the x-axis and y-axis, respectively.

There can be two cases ..

a) y form a triangle

b) a line

in either case point Y will always be outside the circle ...

Statement I tells us the position of point X i.e. it is outside the circle as the radius is 4 and distance betweenn X and P is 4.5

Statement II tells us that point Y is at a distance of 9 units from X.

From both we find Y will cross the the maximum distance between X and the circumference

What you drew makes sense (I drew something similar when I reviewed the problem), but I guess while taking the test the ‘lie in the same plane’ stmt kindof threw me off.
