IMO E)

The sentence definitely requires a 'but' to show the change i.e. from being the best to falling back.So eliminate A) and B)

C) 'in its paying' - awkard construction

D) 'then proving unable to handle the increase in business, falling months behind in paying' - wrong construction. Since this does not bring out the cause and effect construction

E) Correct - no errors - also correctly presents the cause and effect

2/3 splits are on following two issues:

pronoun it
but vs while

coming to pronoun it: what are the noun in sentence: the company
the metro areas?

if metro areas considered then it options gone...

i agree but is good...

but still few more needs to be killed down..for E to be the answer...

Requires But to show the contrast , therefore A and B out.

It and its refer correctly to Health Care Company, also sentence structure should be

Independent clause , but Independent clause.

there is no subject in D and E.

OA pls?

we require a contrasting conjunction...

i was good, but i lost....( similar lines)

C: falling behind in its paying... awkward
D: proving.. wrong tense

E Correct

i was good, but i lost....

where in above sentence both 'I' act as a subject and 'was good' and 'lost' act as a verb.

But in option E I don't see any SUBJECT.

further explanation pls?
