As per my knowledge, it depends on the universities / colleges you apply to. Some universities might not ask for TOEFL or equivalent language test scores if you have completed your undergraduate course in english. For most of the universities, if your native language is not English, TOEFL / IELTS scores are required.

Different Programs, have different policy when it comes to TOEFEL requirements and waivers. Some require you to provide an official certificate attesting to the fact that English was the sole language at your Undergraduate institution, while others require that the official language in your country must be English. Some actually waive TOEFEL simply if you are a US citizen or resident, regardless whether you have studied in the US or not. Duke, for example does not require you to provide any certificate, but evaluates itself whether your language was in english or not. I know atleast one program (Vanderbilt Owen) that automatically grants you a TOEFEL waiver if your Verbal GMAT score is above 90th percentile. Do check with each program, as to their policy, and do so well in advanced as reporting TOEFEL scores does take time.

Maximum universities ask for the TOEFL test scores to check your English fluency. So, go for it, prepare hard, you will definitely crack the exam. Yeah, it is tough but you can crack if you focus more on your reading and writing skills. Take the help of some online writing tutor who can provide you necessary tips on writing skills.
