I went with D.

Killed A B and E because the GMAT loves "that"

Killed C because I think a "that" should be before were, although I don't know if it's an official rule.

Correct form of C can be

that bound for or that originating (Participle parallelism)
that were bound for or that had ('that' is optional)
that were bound for or that orginated

Good question. A typical GMAT question. Well here it goes...


a) bound for or that were (Incorrect as "that" is referring to??)
b) bound for or that had (Incorrect as "that" is referring to??)
c) that were bound for or were (parallel but distorts the meaning)
d) that were bound for or had (Correct)
e) which had been bound for or had ( incorrect, use of which is incorrect. also "had been bound" is not needed)

explanation: first of all the use of "that". remember "that" is acting as a pronoun here.
the sentence talks about Canadian controllers refused ... flights... that were bound for and flights that had originated. There are 2 actions. the flights originated first and then they were bound for some country (united states here). Remember Had (past perfect) is used when we have 2 actions and the action which got completed earlier is shown using "had".Choice D establishes that fact clearly with parallelism.
Hence Choose D
