SPEADING AND INFECTING if modifies LEPROSY then sentence will become illogical.

IMO it modifies " it persists in the popular imagination as a kind of rampant plague". But, want to validate my understanding

"spreading uncontrollably and infecting.." gives us the reason why it persist in this way - adverb of reason. "it" is the logical subject referring to "leprocy"
