The underline portion can be treated as a complete sentence.

(D) Belizeans typically eat their meals with rice, tortillas a rarity
Belizeans typically eat their meals with rice.... can be treated as a complete sentence
tortillas a rarity.... does not describe anything in the sentence. It is not a complete phase, neither can it stand on its own.
To make this sentence correct..... Belizeans typically eat their meals with rice; tortillas are a rarity

(E) Belizeans typically eat their meals with rice, with tortillas as a rarity
with tortillas as a rarity.... is a descriptive phrase.

Another way to look at this, is to rearrange the phases.

D) Tortillas a rarity, Belizeans typically eat their meals with rice.
E) With tortillas as a rarity, Belizeans typically eat their meals with rice.

In D) tortillas and rarity are not compared ... incorrect phrase whereas for E) tortillas is compared to a rarity.
