Let's observe and analyze the options given, and see which of the following is best fit.

In Option A - INCORRECT, words are exactly the same as the ones in the question so asked. It should have bore same meaning but different manner in construction.

In Option B - INCORRECT here, pay attention to the two most prominent verbs "are" and "show". One is active while the other is passive. When they show it means they exhibit but are means they are capable of be doing

In Option C - INCORRECT, this is correct in that it figures out the intention of the writer aptly. It, however, should have been better arranged to show possession by the people assumed to be likely to have. There's a reason where we have "their" in the sentence

In Option D - CORRECT here, unlike in option c above shows possession. It's talking about some particular set of people who could develop heart problems. So, the use of the sentence they are to have is consistent

In Option E - INCORRECT, in the last option here, we feel it would have been a bit more right if it had been arranged to show possession too.
