Main Verb of the sentence is: has identified


A broad range of surveys .................... has identified at least three misconceptions thought to lead to a greater likelihood of unplanned pregnancy, the primary concern of such education.


First of all, subject is "surveys"

"A broad range of" , this is just a modifier for surveys.


b) "greater than likelihood"...this is wrong.

c) the effectiveness of abstinence only education, having..wrong

d) studying the effectiveness of abstinence- only education, and has identified

we don't need AND here

e) they think to be the cause of greater likelihood of unplanned pregnancy



they REFERS to misconceptions or surveys?


Even if we ignore this ambiguity

at least three misconceptions they think to be the cause of greater ....misconceptions is plural so plural verb is needed.....causes
