my initial thought had got me to choice A & B
However then the and in B seems to be a little awkward hence i finally settled down to A;

Can some one put more light on this

one can clearly eliminate C D n E.

but i see problem in both A n B.

A - ....................workers changing jobs.............. sounds very awkward.

B - ....................workers who are changing jobs and otherwise could ..................... parallelism problem.

just wonder what is the source of this question.

Thanks!

The part before and after "AND" has to be parallel here.

This is what B does.

"to people with past health problems" and "to workers who are changing jobs and otherwise could be uncovered for months"

The phrase starting with "establish" should show the results of the health care reform. That's why it is important to use "establishing"

(A) establish standards that would guarantee wider access to people with past health problems and to workers changing jobs who
(B) establishing standards that would guarantee wider access to people with past health problems and to workers who are changing jobs and ( See the parallelism; establishing to people...., and establishing to workers)
(C) to establish standards that would guarantee wider access to people with past health problems and to workers who change jobs that (And "that"refers to job, which is irrelevant)
(D) for establishing standards that would guarantee wider access for people with past health problems and workers changing jobs who
(E) for the establishment of standards that would guarantee wider access for people with past health problems and workers who are changing jobs that ( Establishment is wordy than to establish. MGMAT V>A>N rule. And "that"refers to job, which is irrelevant)

how can OA would be B?
establishing standards would guarantee wider access to people ...and standards would guarantee wider access to workers ...

It makes sense.
