got 640 GMAT.
Q-47,
V-31,

Is it good enough for top 5 particulary- Harvard, MIT, Stanford?

13 years Work experience. already an MBA with Engineering background.

You will need very strong extra curricular activities and entrepreneurial work experience. Basically, with a score like that, you need to really separate yourself from everyone in the field. If you own your own business and do a lot of volunteering and have global experience, you have a chance. I hate to say it, but without stuff like that, it will be tough to get in with that score.

For comparison purposes, I got a 760 on the GMAT and I don't think I can get into Harvard unless I really beef up my Resume in the next year or so.

Most of the top 10 programs in the US will not accept applicants who already have an MBA.

Actually most of them will admit. Very few wont. You'll also have to explain the need for a 2nd MBA, particularly a 2yr MBA when after 13yrs you could be going for a 1yr MBA to places like INSEAD,IMD which are ranked top5 for the 1yr program.

The best thing would be to mail the top5, explain your situation and see what they recommend. Top schools love hearing from potential candidates especially those who can be really competitive. Your work-ex and salary will only boost their averages not to mention you would have great value-add in the class. If possible, try meeting them at the MBA fairs that happen.
