I never seen "effort for+time " when we talk of time period. so, it is posssible that "effort for+time period " is wrong
but I am sure "effort to do" is idiom. so "effort ...that has subjected" is not idiomatic. D is wrong.
in the idiom "effort to do", the agent which cause "to do" is not present, so "to do" is correct. we do not need to show the agent of "to do".
I thing basis idiom is the topic of this offical SC question.
