John Smith provides information on the conditions that lead women to a gynecologist, and he notes that these conditions can, and sometimes are, used in the promotion of surgery which is not needed.

(A) and he notes that these conditions can, and sometimes are, used in the promotion of surgery which is not needed
(B) noting that these conditions can be, and sometimes are, used to promote needless surgery
(C) and notes that they could and sometimes are used unnecessarily in promoting surgery
(D) sometimes promoting needless surgery
(E) which they use sometimes to promote unnecessary surgery

OA - B

Go with B.
Reasons:

A: Case of Parallelism. Bifercate the sentence, and you will get the error here. "and he notes that these conditions can.....used in the promotion of surgery which is not needed" is incorrect. "Can be" is correct form. Ignore A.

C: "and notes that they could and sometimes are used unnecessarily in promoting surgery "...same error as A. "Could (be) used....be is missing."

D: sometimes promoting needless surgery
This is modiier error. What promotes needless surgery? "information provided by John" or "Conditions". Ignore this.

E: Which refers to gyno...ignore this.

B is correct I guess.
