How do recommendation letters work?

I do not have very much experience with recommendation letters (except high school --> college). I've read in articles where an applicants consultants will edit a recommender's essay (is this even possible?) or applicants will write their own recommendation and have someone else rubber stamp it.

Does anyone have a clear guideline laying out exactly how these are done?

Do not pay attention to any of these stories or claims. Letters of recommendation are easy because you don't have to write them. The recommender will submit the letter him/herself. You should sit with your recommender to go over your goals though and make sure that you have their support.
