The underlined phrase is a modifier; it is used as an adjective to describe the noun that immediately follows it. In this case, the phrase incorrectly modifies "the prosecuting attorneys", which doesn't make any sense.

A - The underlined phrase incorrectly modifies the noun

B - It doesn't have a clear reference

C, D - wordy and awkward

E - This phrase correctly modifies the noun. Hence, this should be the correct answer

Yes the OA is E
Same explanation was provided by Kaplan
But I still don't understand whats wrong with A

Based on accounts of several witnesses, the prosecuting attorneys.... the prosecuting attorneys cant be based on accounts... thats the error with A... they used the accounts...

the prosecuting attorneys used the accounts to formulate is correct...

Let me modify A slightly to make it possibly correct.

"Based on accounts of several witnesses, the real story of what actually happened will finally be told."

The real story was based on accounts of several witnesses.

In A: "Based on accounts of several witnesses, the prosecuting attorneys...," flip flopping the modifier would make it nonsensical:

"The prosecuting attorneys was based on accounts of several witnesses..."
