Almost a decade after New York State passed laws to protect patients by reducing the grueling hours worked by medical residents, twelve hospitals have been investigated by state medical officials, finding that all twelve consistently break the laws, many residents work longer than 24 hours straight, and that more than half the surgical residents work more than 95 hours a week.

What happened after a decade......An investigation has reveiled something...

Hence the independent clause shud start with "An investigation"..Eliminating A and D.

An investigation (of 12 hospitals.....) has/have....it shud be "has"....eliminating B.

"AND" is absent in option "E". eliminate it.

Left with "C".

Perfectly parallel.

aninvestigation found 1) that all twelve hospitals......
2) that many residents .....
AND 3) that more than.....

is the OA "C".

The correct answers seems to be C.

A lacks parallel construction - missing a "that" before "many residents"
B also lacks parallel construction by using "consistently breaking the laws, that many residents...."
D also lacks parallel construction by saying "with many residents working longer than 24 hours straight, and
more than half the surgical residents work "
E - same problem by saying "found that all twelve consistently break the laws, that many residents work longer than 24 hours straight, with more than half the surgical residents working

But I don't understand the construction of the sentence
"An investigation........has found"
How can investigation being a non-living thing can find something
State medical officials have found it.

is there any thing wrong with this sentence, though preparation (non-living thing) takes me .....

Would be better to say:

My serious preparation will take me to a score of 780.

But the main idea is correct - your preparation can take you to a score of 780
