I am confused between C and D. Here we need to check the meaning and usage of the verbs also, not plainly parallel structure. Tough one.

Going by your logic..why not C. Parallel structure there also...Impose...Require...Spend?

IMO C

Here, there is a parallelism with AND.
Between B and C.
In B, sentence reads like, non union members impose ...
But, it is the insurance plans that impose ...
