Why is A correct ..isnt between x and y idiom necessary ?

There are two takeaways here.

One is that we are looking for the best answer choice, and so if all of the answer choices do no conform to a rule you believe to be relevant, then the apparent breaking of that rule is not a decision point for determining the best answer to the question.

That being said, actually in this case the use of between does not constitute an issue anyway.

While, yes, between can be used to create the idiomatic structure, between x and y, that is not the only correct way to use between in the context of a comparison or discussion of a comparison.

The expression in choice A, comparisons between cities of different sizes, is fine.

Be careful about how you think about and apply rules in SC. Getting too caught up in rules and misapplying rules are ways people often run into problems in GMAT sentence correction.[/i]
