I couldn't understand anything from this one. From Princeton Review Online free test:

Believed to have originated from local hospitals, oceangoing garbage scows dump medical refuse that may provide an explanation for the existence of unacceptable levels of pollution at several local beaches.


A) Believed to have originated from local hospitals, oceangoing garbage scows dump medical refuse that

B) Oceangoing garbage scows dump medical refuse that is believed to have originated from local hospitals, and this

C) Oceangoing garbage scows dump medical refuse, believed to have originated from local hospitals, that

D) Originated, it is believed, from local hospitals, an oceangoing garbage scow dumps medical refuse and

E) Originating, it is believed, from local hospitals, oceangoing garbage scows dump medical refuse and

Let's figure out the error in A:
Original msentence means Oceangoing garbage scows has been believed to have originated from local hospitals. But that is NOT the intended meaning.

Sentence intends to say, Medical refuse has originated from local hospitals.

Coming to Options :
In B ; usage of "that" properly refers to medical refuse & the word "this" refers to the action done by Oceangoing garbage scows .

in B - the information coming after "that" is essential to the sentence, therefore "that" is needed.

Here this correctly refers to the action -my answer

C) Oceangoing garbage scows dump medical refuse, believed to have originated from local hospitals, that
Believed to have..correctly modifies refuse but this sentence does not have main verb . it reads something like refuse that might.. Eliminate

D) Originated, it is believed, from local hospitals, an oceangoing garbage scow dumps medical refuse and
and might provide .... Eliminate

E) Originating, it is believed, from local hospitals, oceangoing garbage scows dump medical refuse and
Same as above

i believe that you are right, and that the question is wrong.

in my experience of formal english, and certainly in my experience of gmat questions, it is, in general, wrong to use "this" as a standalone pronoun.

like you, i have only seen "this" used correctly as an adjective.
Ron has been teaching various standardized tests for 20 years.
