Initially I also selected B.

But actually B has a modifier issue.

Forbidden by Church law to marry, it was not unusual for a priest during the Middle-Ages to sire a family.

only E has no issue.

I think there is no problem with "it" , otherwise even E would have a problematic...rite??

The problem is with no contrast being present in B, the same is mentioned in the answer explanation.

The explanation says..."the fourth choice changes the intended meaning of the original by implying that there was a single priest who sired several families."
I could not understand how the sentence intends that... I would perhaps like some expert to shed light on this!

A,B: Modifier Problem (Who was forbidden)
C: Pronoun problem (they..priest - should be priests)
E: What was unusual for priests - to sire families (was corresponds to families, plural)

So D, Also in D subject starts early (Although a priest..than postponing to the later part of the sentence)

rewrite this sentence as

It was not unusual for a priest during the Middle-Ages to sire a family, forbidden by Church law to marry.

now Forbidden by Church law to marry doesn't clearly refers to priest . so seems erroneous to me. my 2 cents.
i never have a "TO -DO" list and i end up having a lot of "NOW WAT TO DO ?" lists

E does not have a modifier issue.

Some concept here :
Actually there are two ways to remove an opening modifier issue (as in B)

1. Instead of modifier, create a clause. [ E is doing that, here they can refer to priests only ]
2. put the right thing just after the modifier. we can write B as
e.g. Forbidden by Church law to marry, priests ......

D is clearly wrong, with the same reason whatever you have written.
Read it again and you will come to know.

You can quickly eliminate A & B, because it is ambiguous as to who has been forbidden. Was "it" forbidden or was it the priest?

C can be eliminated because the two dont agree in number. "they were forbidden".... "a priest"

D is eliminated because the meaning changes. "a priest was forbidden" means that the sentence is simply about one specific priest, which is not the intended meaning.

That leaves E. This clears the number issue as it uses "they" and "priests". The use of although is proper here as well.
