IMO C.

By the way, if the underlined portions are correct, choices A, D, and E would each contain two consecutive instances of "would".
I'm really old, but I'll never be too old to become more educated.

the OA is E
option E correctly maintains parallelism with the use of 'that' and 'would' indicates future from a past action's perspective.
were established - past tense
would - future from past's perspective

I have duly added this point to my flashcards..

The fishermen had been warned of the storm just 48 hours before it struck.
The fishermen had been warned about the storm just 48 hours before it struck. INCORRECT IDIOM

The new swine flu screening rules were issued to ensure that all patients must be vaccinated before they board the airplane.

Correct : The new swine flu screening rules were issued to ensure that all patients BE vaccinated before they board the airplane.
Is the above version correct? The 'be' form is used to indicate a command ( passive voice above is clumsy but i couldn't pen down a better example)

The following is correct:

The new rules were issued in order to ensure that all patients would be vaccinated.

The issue here is tense. That all patients would be vaccinated is an event happening in the past after another past event (the new rules were issued). The verb would is needed in order to make the sequence of events clear.

As mentioned above, Warned/Warn should be followed by "of" not "about". So, C is wrong.
