A) Added to worries about budget cuts and cost of living increases
B) Added to budget cuts and increases in the cost of living
C) In addition to worry about cuts int he budget and as the cost of living increases
D) Added to their worrying about budget cuts and cost of living increases
E) In addition to worrying about budget cuts and increases in the cost of living

The answer is E

A) incorrect. budget cuts and cost of living increases are not parallel
B) incorrect. the administrators are getting added to the cuts and cost of living
C) incorrect. again not parallel. Same as A. as cost of living is awkward.
D) incorrect. again the administrators are getting added to the cuts and cost of living
E) correct. modifies the administrators correctly

one can rephrase the sentence so that it makes sense!
the administrators of public health care facilities mush contend with the possibility of a strike by employees, in addition to worrying about budget cuts and increases in the cost of living

A) "cost of living increases" should be a noun instead (eg increased costs of living) to be parallel with worry about budget cuts
B) Noun phrases stand alone: grammatically incorrect
C),D) Parallelism issue - same as A)

B) Added to budget cuts and increases in the cost of living - same as A
C) In addition to worry about cuts in the budget and as the cost of living increases - The 2 things, cuts in budget and increase in cost of living- not parallel
D) Added to their worrying about budget cuts and cost of living increases - similar to C
E) In addition to worrying about budget cuts and increases in the cost of living - CORRECT

As many of you have noticed, we have a subtle 3:2 split here between "Added to" and "In addition to". The distinction between these two is not so much idiomatic as logical. The past participle "Added to" would describe the noun/subject coming immediately after the comma - "the administrators" - which is completely nonsensical in this cases, since "the administrators" cannot be "added to" anything. Eliminate A, B, and D

We need to say that these poor administrators were faced with many worries - contending with possibility of a strike by company employees, and worrying about budget cuts and increases in the cost of living - we need an 'In addition to' to aggregate these various problems faced by the administrators.

Between C and E, C breaks the parallelism - "worry about cuts in the budget" (noun) and "as the cost of living increases" (clause) are not in parallel with each other. Eliminate C

This leaves us with E as the correct answer.
