For 15 pounds of weight 2 CC of medicine is required,

so for 120 pounds 2*120/15 = 16 cc of medicine is required

Doctor prescribed 18 cc

% extra = 2*100/16 = 12.5

Why is it not 2*100/18 ?? Do we normally treat this kind of question like a
%increase, where the denominator is always the lowest of the 2 values?

Otherwise, I would have had an equation like this,

x/100 * 18 = 18-16.

you will calculate the increase over what the dosage should have been i.e. over 16

since overage is 2 the eq is 2/16 x 100 = 12.5
