a) correct

b) sub - verb does not agree

since entourage is a showing a group so "requires" satisfy it
also we will have to use "she" and not "her "

Option A IMO.
The point is that here the two subjects are connected by AND,hence requiring a 'require'.Had they been connected by the connector 'with',I would have opted a singular 'requires'.

Technically 'and' guarantees a compound subject and hence the following verb must be in the plural;
however here, when we say that 'she and her entourage require a large fleet of vehicles',
it could mean that 'the queen' and 'the entourage' both require 'one' large fleet of vehicles.

Or it should have been something like they both require one fleet each, which obviously is not the intend of the sentence.
This is as far as i can get.

IMO A.
though entourage is a collective noun , additive and forms a plural subject and takes a plural verb require.

My pick is 1.
2- plural verb required , so wrong
3 and 5 are wrong usage.
4 - that is not required here does not fit in the sentence structure..
