You can feel that the sentence requires either X or Y structure.

If we use either X or Y structure, then both X and Y need to be parallel. Only D maintains the parallelism by using "to have had miscarriages" and "to have borne children with heart defects".

I think the verb here is "have borne". "Children" is not part of the verb. I think you are confused between the usage of "borne" and "born". "Borne" is past tense of "bear".

In A and B, it is been illogically suggested that children had miscarriages too.
In C and E, parallelism is violated. Correct form: "to have either X or Y" or "either to have X or to have Y". Furthermore, C illogically suggests that children also had miscarriages.
D maintains parallelism through "either to have X or to have Y" and meaning is also clear. Hence, D is the answer.
